Inter-institutional variability in kidney dosimetry during Lu-DOTATATE therapy in Japan

This study evaluated inter-institutional variability in kidney dosimetry using common Lu-SPECT/CT datasets from the SNMMI Dosimetry Challenge. Ten Japanese centers calculated kidney volumes and absorbed doses with various organ- and voxel-based software. Manual VOI contouring caused large variability (CV up to 16.9%), reduced to 7.4% using common VOIs. Voxel-based dosimetry yielded higher doses but reduced variability, highlighting the need for algorithm standardization to ensure reliable Lu-DOTATATE kidney dosimetry in Japan.
