Optimizing lens and organ dose evaluation in head CT examinations using Monte Carlo simulation; Influence of gantry tilt and scan range

近年、くも膜下出血や脳腫瘍、頭部外傷の診断のためにCT装置を用いた頭部撮影が頻繁に行われている。一方、頭部に位置する水晶体は、放射線被ばくにより白内障リスクが高まることから、被ばく線量の低減が重要な課題となる。そのため、CT装置のガントリ(X線の発生部と検出器)の角度を調整する撮影法が水晶体線量低減の手段として注目されている。しかし、具体的な角度や撮影範囲はプロトコールで定義されていなかった。本研究では、原子力機構が開発したJF103ファントムとPHITSコードを用いて頭部CT撮影で放出されるX線の挙動を模擬し、体内線量分布を解析した。その結果、ガントリ角度と撮影範囲を最適化することで水晶体への線量を有意に低減できることを定量的に示した。本知見は、頭部CT撮影において水晶体防護を最適化するためのプロトコール改訂において重要となる。

In recent years, head CT scans have been used to diagnose subarachnoid hemorrhage, brain tumors, and head trauma. However, because the lens located in the head is at increased risk of cataracts due to radiation exposure, reducing radiation dose is a key challenge. Therefore, adjusting the angle of the gantry (the generator and detector of X ray) of the CT scanner has been used as a means of reducing lens dose. However, the specific angle and imaging range have not been defined in the protocol. In this study, we simulated the behavior of X rays emitted during head CT scans using the JF103 phantom and PHITS code developed by the JAEA, and analyzed the dose distribution in body. Our results quantitatively demonstrated that optimizing the gantry angle and imaging range can significantly reduce the lens dose. This finding will be important for revising protocols to optimize lens protection during head CT scans.
