Age-dependent evaluation of organ and effective doses in pediatric full-spine radiography; Influence of anteroposterior and posteroanterior projection and copper filtration using Monte Carlo simulation

小児脊柱側弯症においては、経過観察を目的として全脊椎X線撮影が繰り返し行われることから、放射線感受性が高く、胴体前部に位置する臓器の被ばく線量が増加する傾向がある。そのため、撮影プロトコルの最適化が必要とされていた。そこで、本研究では、小児フロリダ大学ファントム(5、10、15才)及びPHITSを組み合わせることにより、全脊椎X線撮影を模擬し、銅フィルタの有無及び撮影方向(前方:AP、後方:PA)が臓器線量に及ぼす影響を解析した。その結果、PA撮影の場合、胴体後部に位置する腎臓や赤色骨髄の線量はAP撮影と比較して増加するのに対して、胴体前部に位置する乳房や甲状腺の線量は約80-93%減少し、実効線量についても半減することが示された。加えて銅フィルタの利用により、臓器線量は約5-19%減少した。以上のことから、小児全脊椎X線撮影においては、年齢に応じて変化する臓器位置を考慮しつつ、銅フィルタを用いたPA撮影を行うことが、被ばく線量低減のための撮影プロトコルとして有用であると考えられる。
In pediatric scoliosis, repeated whole-spine radiography is performed for follow-up, resulting in increased exposure to organs located in the anterior torso. Therefore, optimizing the radiography protocol is essential. In this study, we simulated whole-spine radiography using pediatric University of Florida phantoms and PHITS. We analyzed the effects of a copper filter and the direction of radiography (anterior: AP, posterior: PA) on organ doses. Compared with AP radiography, PA radiography increased doses to the kidneys and red bone marrow located in the posterior torso, while doses to the breast and thyroid located in the anterior torso were reduced by approximately 80-93%, and effective doses were also halved. Furthermore, the use of a copper filter reduced organ doses by approximately 5-19%. These findings suggest that PA radiography using a copper filter, while considering organ positions that change with age, is a useful radiography protocol for reducing exposure doses in pediatric whole-spine radiography.
