This report describes an investigation and countermeasures of the unexpected reactor shutdown of the TRACY facility at the Japan Atomic Energy Research Institute on June 17, 2004, caused by a malfunction of a safety rod of the facility. As a result of the investigation, the principal cause of the malfunction was ascribed to a drop in attraction of a binding magnet for the safety rod by a small piece of a foreign material. The following countermeasures were taken against its recurrence: prevention of incidence of foreign materials, reduction of the chance of intrusion of foreign materials into devices, and certain detection of foreign materials. This lesson learned through the experience is expected to be shared with the staffs concerned in reactor operation and maintenance.

Fuel pellets may be scattered, in the accident of Fukushima Daiichi Nuclear Power Station, without melting in the containment vessels. Precise information of composition of the burnt BWR fuel assemblies is necessary to study the criticality control method for the scattered fuel pellets. In this study, distribution of residual U enrichment was evaluated, for each of the reactors, using a model of a fuel-rod array in a fuel assembly.

Two proposal notes were written by the Japan Health Physics Society in terms of the issues on radiation protection considering the arguments in many academic symposiums after the accident in TEPCO Fukushima-Daiichi NPP resulting from the Great East Japan Earthquake of March 11, 2011. These proposal notes were referred in some symposiums, and the follow-up activities are needed for collecting the chronological information after the Fukushima accident and for considering these proposal matter in detail. After the proposal notes were published, quite many activities for radiation protection were performed. In this professional workshop, the chronological information was collected and easily evaluated concerning with the proposal. In this presentation, the outcome was introduced.
